Recently, we sent out deficiency notices for older Out of State applications. For the applications you received a deficiency notice on it is because of an identity verification issue.In order for Ohio to process the application further, the identity of the applicant will need to be verified. Without this verification we can not certify an application. We appreciate your understanding and cooperation as we work to improve Ohio WOTC.
Upon entry of an application, if the FEIN does not automatically populate, please enter the entire FEIN and tab over to the next field to complete the application.
Our office has recently noticed that some of the supporting documentation being provided for the ex-felon target group is not an acceptable form of documentation. Please remember supporting documentation must clearly show the conviction or release date. I have provided a list of acceptable documents below:
State of Ohio Ex-felon verification form
Parole Officer's Name and signed Statement on Letterhead
Correctional Institution Records
Court Records from the county website
Federal Bureau of Prisons (BOP) website
Release Papers from ODRC (received in their exit packet)
Releasee ID from ODRC (received in their exit packet)
Identity on all applications is verified. If you receive a Deficiency Notice asking for ID verification we accept any of the following
A government issued ID such as
Drivers License (be mindful this may not verify SSN)
Passport
State ID (be mindful this may not verify SSN)
or a
Social Security Card (be mindful this does not verity DOB)
Birth Certificate (be mindful this does not verity SSN)
or Lexis Nexis Report that does not identifying information redacted.
